I'm in an industry that involves long hours - at one company I worked, one of these was left behind by someone and I chose it over the standard desk chair. I would even take this thing over an Aeron.
Bought the large size despite that I'm an average sized man because I like to keep it less than fully inflated for comfort; it's inflated so it comes just up to my desktop.
Not the intended use, perhaps, but it works for me! Back support and seat comfort are no longer an issue here, and my body makes micro-adjustments throughout the day due to the roundness of the ball (and the occasional desire to bounce a little).
Also, as furniture this one's not so ugly as many other more psychadelic and hippie-ish ones...
